MINIMUM QUALIFICATIONS

EDUCATION, CERTIFICATION, AND/OR LICENSURE

• Must possess current license or trainee license as required by state board where services will be provided:

MD: Pharmacy Intern or Pharmacy Technician license through Maryland Board of Pharmacy OH: Pharmacy Intern or Pharmacy Technician license through Ohio Board of Pharmacy WV: Pharmacy Intern License (Registration) or Pharmacy Technician license through West Virginia Board of Pharmacy PA: Pharmacy Intern license through Pennsylvania Board of Pharmacy

• Must be enrolled in or accepted to an accredited School of Pharmacy OR graduate of an approved college of pharmacy or graduate of a foreign pharmacy who has obtained Foreign Pharmacy Graduate Examination Committee certificate for purposes of getting required intern hours OR qualified graduate who is awaiting examination for licensure OR individual participating in a residency or fellowship program.
